Computer Equipment request form (returned to IT team)

Please note the following points before filling the forms of equipment request.

1. A computer will be shared by 4-5 students (maximum).
2. IT Support team assume 1GB hard disk space can store 5 minutes DV video clips.
3. Students have to submit the form of editing and shooting equipment needs to production center and IT team during Sept 5-30, 2005. Students will submit the form ONCE ONLY this year. Therefore, students need to calculate accurately how much e-money and hard disk space they need. For documentary students, they can submit their form as early as Sept. 5 and start using equipments, and for others, students can submit their form as late as Sept 30, so that they have more time to think about how much e-money they really need.
4. After Feb. 1, 2006 studentsˇ¦ e-money will be progressively deducted in the following way:
- February 1: 15% deduction of existing e-money
- February 28: 30% deduction of existing e-money
- March 31: 30% deduction of existing e-money
5. If the student requests more than the approved e-money or hard disk, such request have to be vetted by the FYP Administrators and the IT and PC representatives (i.e. Joshua, Garry, Linda, Phoebe and Kim), called the FYP management group below. The student and involved supervisor have to meet the group to explain why extra resources are needed for the project. The meeting will be held from 4:30pm to 6:30pm, October 18, 2005.
6. Once FYP requested resource is approved, additional resource request would NOT be entertained normally. If the student thinks that it is a must to add resources, they needs to ask their supervisor to come to have a meeting with the FYP group. Maximum additional resources would only be 25 per cent of the original requested resources.
7. Installation students need to submit/confirm your presentation equipment list before Apr. 18, 2006.
